Role overview
What you'll be stepping into
Proactively identify eligible patients across all hospital units, including medical, surgical, ICU, oncology, geriatrics, neurology, orthopaedics, and palliative care.
Coordinate comprehensive discharge planning for patients transitioning to home care.
Work collaboratively with consultants and multidisciplinary teams to develop individualized home care plans.
Educate patients and caregivers on available home care services and facilitate informed decision-making.
Coordinate assessments, equipment, medications, and follow-up services before discharge.
Grow home care referrals and revenue through strong internal stakeholder engagement and service promotion.
Monitor quality indicators, patient outcomes, readmissions, and customer satisfaction.
Build partnerships with clinicians to position home care as the preferred post-discharge care option.
Prepare regular reports on referral trends, conversion rates, patient outcomes, and business performance.
Drive continuous improvement initiatives to expand the reach, quality, and sustainability of the hospital's home care programme.
This role requires a highly organized healthcare professional with excellent clinical knowledge, relationship management skills, strong business acumen, and a passion for delivering patient-centred care beyond the hospital walls.
It plays a pivotal role in extending the hospital's care continuum while improving patient outcomes and supporting sustainable organizational growth
Requirements
Minimum of a Diploma in Nursing.
At least 2 years of relevant nursing experience.
Demonstrated understanding of business development, including the ability to identify opportunities for service growth, build client relationships, and support revenue generation.
